Dear parents,

Please find on this page all the documentation, information and forms you need to read and/or complete before your child starts school with us.

We know it is a lot to take on, so please do not hesitate to call us at the office for any query: 0121 464 5500.

    Photos and videos consent form

    In order to use photograph and video footage of your child we will need your consent. It is now a legal requirement that we seek positive consent for use of photograph and video footage in specific circumstances, rather than as one overall consent, so you can choose the circumstances you are consenting to.
    Please note that if you do not fill in this form indicating which options you are consenting to we will not be able to use your child’s photograph or video footage in any circumstances other than where there is a statutory reason to do so, such as safeguarding, where your consent is not required.

    school meals

    Everyday, our cook, Mrs Guiden, prepares delicious, nutricious and healthy meals for most of our children. Children in Reception, Year 1 and Year 2 can have their daily lunch for free in school.

    off-site visits

    From time to time, teachers will organise short visits for their class, within walking distance from school. These visits will be part of his/her school work and he/she will be supervised on each occasion.

​​School meals are free for all Reception age children. We would ask that you complete the application form below if you are in receipt of any of the following:
  • Income support
  • Income based jobseeker's allowance
  • Support under part VI of the Immigration and Asylum Act 1999
  • Child tax credit (provided you are not also entitled to working tax credit and have an annual gross household income of no more than £16,190)
  • The guaranteed element of State pension credit
  • Income related Employment and support allowance
  • Working tax credit run on - paid for 4 weeks after you stop qualifying for working tax credit
This will potentially allow school to receive extra funding from the Government to use in support of teaching and Learning (Pupil Premium scheme).

​The ethos of this school is Catholic. The school was founded by the Catholic Church to provide education for children of Catholic families. Whenever there are more applications than places available, priority will be given to Catholic children in accordance with the over-subscription criteria listed in the school’s published admission policy. The school is conducted by its governing body as part of the Catholic Church in accordance with its Trust Deed and Instrument of Government and seeks at all times to be a witness to Our Lord Jesus Christ. As a Catholic school, we aim to provide a Catholic education for all our pupils. At a Catholic school, Catholic doctrine and practice permeate every aspect of the school’s activity. It is essential that the Catholic character of the school’s education be fully supported by all families in the school. We therefore hope that all parents will give their full, unreserved and positive support for the aims and ethos of the school. This does not affect the right of an application who is not Catholic to apply for and be admitted to a place at the school in accordance with the admission arrangements.
A Certificate of Catholic Baptism or Reception into the Catholic Church is required in order for the Governing Body to give the correct priority to an application.
Failure to provide evidence of Catholic Baptism or Reception in the Church may affect the over-subscription criterion that the child’s name is placed in.


The information below is about our wrap around care at St Martin de Porres. We provide both breakfast club and after-school club for our pupils. Numbers are limited. If you do think your child will be requiring any of these clubs, please contact us at the office 0121 464 5500:
